Mayim Group (ABN 60 698 761 607), trading as Mayim Care, collects this information to assess your application, verify your identity and work rights, complete the screening checks required of NDIS workers, contact your referees, and meet our record-keeping obligations as a disability support provider.
Sensitive information. Some of what we collect is sensitive information under the Privacy Act 1988 (Cth) — including your NDIS Worker Screening Check, Police Check and Working with Children Employee Check details, and health and pre-existing injury information. We collect this only with your express consent, which we ask for at the end of this form.
If you don't provide it. We cannot progress your application. A valid NDIS Worker Screening Check clearance is a legal precondition for working in a risk-assessed role.
Who we share it with. Your referees and previous employers; the NDIS Worker Screening Unit and the NDIS Quality and Safeguards Commission; the Victorian Disability Worker Commission where applicable; registered training organisations and the USI registry for qualification verification; licensing authorities (VicRoads / Transport for NSW); and our service providers, including our form platform (Tally), HR and payroll systems, and IT and hosting providers. Some of these store data outside Australia.
