03 · Statement of Standards — Confirmation

I have read the Statement of Standards — what good care looks like (MC-QLD-DOC-03, version 1.0) in full, I understand it, and I agree to provide care that meets it. I understand the standard of care every child in care is entitled to under section 122 of the Child Protection Act 1999 (Qld). I understand that where a concern is raised the department may conduct a standard of care review in which I may be named as the person allegedly responsible; that being named is not a finding; that Mayim Care must notify the department of the history change using LCS Form 6 whether or not the matter is substantiated; and that being named is a change in my child safety history which I must disclose to Mayim Care immediately. I understand that this form and my signature are a legal record kept on my personnel file.
