08 · Employee Handbook — Confirmation

I have read the Employee Handbook (MC-QLD-DOC-08, version 1.0) in full, I understand it, and I agree to comply with it. I understand that I must tell Mayim Care immediately of any change to my criminal history, child safety history, blue card or exemption card status, domestic and family violence history, or any charge for drink driving, drug driving or dangerous driving; that Mayim Care must notify the department of these changes using LCS Form 6; and that if my blue card or my Child Safety and Personal History Screening recommendation lapses or is suspended I must stop work that day. I understand that this form and my signature are a legal record kept on my personnel file.
