This form is the Independent Contractor Agreement between Mayim Care and you. It is for psychologists, occupational therapists, physiotherapists, speech pathologists, nurses, behaviour support practitioners and every other allied health or professional service provider who works with Mayim Care participants as an independent business, not as an employee.
