04 · Mandatory Reporting and Reportable Conduct Procedure — Confirmation

I have read the Mandatory Reporting and Reportable Conduct Procedure (MC-QLD-DOC-04, version 1.0) in full, I understand it, and I agree to comply with it. I understand that I am a mandatory reporter under section 13E of the Child Protection Act 1999 (Qld); that I must report a reasonable suspicion of significant harm from physical or sexual abuse where a parent may not be able and willing to protect the child, and a reasonable suspicion that a child in our care has suffered, is suffering or is at risk of harm; that I must make the report myself and telling my supervisor does not discharge my obligation; and that if I am instructed not to report, I must report anyway. I understand the Reportable Conduct Scheme under the Child Safe Organisations Act 2024 (Qld) applies to my conduct and that I must cooperate fully with any investigation. I understand that this form and my signature are a legal record kept on my personnel file.
