Mayim Care — NDIS risk assessment for supports delivered outside the participant's home.
This assessment is completed by a Mayim Care assessor before a participant is supported to access the community, and is reviewed at the intervals set at the end of the form. It exists to satisfy the NDIS Practice Standards (Core Module) — Rights and Responsibilities (independence and informed choice; violence, abuse, neglect, exploitation and discrimination), Provider Governance and Operational Management (risk management and incident management), and Provision of Supports (safe support delivery and responsive support provision) — together with Mayim Care's duty of care under the NDIS Act 2013 (Cth), the Disability Act 2006 (Vic), and Victorian occupational health and safety obligations owed to workers.
Dignity of risk. Identifying a risk is not a reason to refuse a support. The purpose of this form is to enable the activity with the right controls, and to record the participant's informed choice where they decide to accept a risk. A decision not to proceed must be justified in writing in the Residual Risk and Decision section.
