I have read the Privacy Policy — Queensland edition (MC-QLD-DOC-06, version 1.0) in full, I understand it, and I agree to comply with it. I understand that disclosing information about a child in care to a person not entitled to receive it may be an offence under the Child Protection Act 1999 (Qld); that I must not access a young person's information unless I need it for my role; that I must not store any young person's information on personal email, personal cloud storage, personal messaging apps or any artificial intelligence tool; and that I must report any suspected data breach to privacy@mayimcare.com.au immediately. I understand that this form and my signature are a legal record kept on my personnel file.
