This form is completed by a Mayim Care staff member together with the participant (and their nominee, family or advocate where the participant wants them present). It is the single intake record for a new participant and forms part of the participant's file.
You do not have to finish this in one sitting. Use the Save and continue later link at the bottom of any page. The NDIS Practice Standards require that participants have enough time to consider their options at each stage — pausing between sections is expected, not a problem.
Before you start, tell the participant: why the information is being collected, who will see it, that they may decline to answer any question, that declining will not stop them receiving supports, and that they can ask for a copy of anything recorded here. Privacy consent is captured formally in Section 18.
