This assessment is completed by a Registered Nurse with the participant present. It establishes the clinical baseline for the nursing care plan, determines which nursing tasks may safely be delegated to support workers, and must be completed before community nursing supports commence.

Before you begin: confirm the participant's identity, explain what the assessment involves, and obtain consent for physical examination. The participant may decline any component without affecting their supports.
