I have read the Welcome Pack (MC-QLD-DOC-00, version 1.0) in full and I understand it. I understand that I cannot begin work, paid or unpaid, until I hold a valid blue card or exemption card linked to Mayim Care through the Blue Card Services organisation portal and a current positive Child Safety and Personal History Screening recommendation from the Central Screening Unit — that these are two separate checks and a blue card on its own is not enough. I understand that mandatory reporting, my duty to disclose changes to my history, and my confidentiality obligations apply from my first shift and continue after my last. I understand that this form and my signature are a legal record kept on my personnel file.
